Total Gastrectomy for Gastric-Cardiac Cancer



Abstract
One hundred and fifteen gastric cancer patients were operated with total gastrectomy at Hanyang University Hospital during 5 years from January 1986 to December 1990. Among them 59 patients(50.1%) could be followed. The follow-up patients were
analysed
retrospectively and the results are;
1) During 5 years, 710 cases of gastric cancer were operated. Among them. 115 cases(16.5%) were operated with total gastrectomy, and 59 cases could be followed.
2) The total gastrectomy ratio was increased from 9.32% in 1986 to 23.49% in 1990.
3) The most prevalent age group was in the 6th decade(33.9%) and male to female ratio was 1.46 : 1.
4) The stage distribution: I(13.5%), II(15.3%), III(49.2%), IV(22.0%).
5) The duration of preoperative symptoms showed some relation to the survival rate but statistically not meaningful.
6) Perioperative transfusion and splenectomy did not affect the survival rate.
7) The length of proximal surgical margin affected the survival rate but statistically not meaningful.
8) Postoperative 30 months showed some meanings because survival rate showed nearly same value after this periods.
9) According to the Kaplan-Meier product limit probability, the survival rate was 0.81 in the low stage group(stage I+II) and 0.27 in the high stage group(stage III+IV).
10) The depth of tumor invasion and the degree of lymph node metastasis were the most prognostic factors.
